Instead, it is spreading into new regions of the United States. This is happening even as the concentration of the drug within each dose appears to be falling. For families, educators and prevention advocates, the findings offer a sobering reminder. The illicit drug supply remains unpredictable and dangerous.
How Xylazine Fentanyl Contamination Has Changed Since 2023
Researchers from Johns Hopkins Bloomberg School of Public Health analysed more than 42,000 urine drug test results. The samples were collected between March 2023 and September 2025. Every sample included in the study tested positive for fentanyl. Each was also checked for xylazine, a veterinary sedative never approved for human use. Consequently, the study offers one of the clearest pictures yet of how this dangerous adulterant is moving through the country.
Overall, the results show a troubling pattern. In the eastern United States, xylazine detection rose from 11.11 percent to 19.63 percent between the first and last months of the study. Meanwhile, in the west, detection rose even more sharply. It climbed from 0 percent to 13.46 percent over the same period. Odds were 55 percent higher for every tenfold increase in fentanyl concentration.
