Local governments across the United States are turning to wastewater drug testing. The goal is to spot dangerous substances before they cause widespread harm. From small school districts to entire cities, officials are collecting sewage samples and analysing them for traces of illicit drugs. Many hope it will give communities an early warning system against emerging threats.
In Tempe, Arizona, city workers rise before dawn. They gather samples from eleven sewage monitoring stations scattered across the city. The collected liquid travels to a new municipal laboratory, where chemists search for substances such as fentanyl and xylazine. Results appear on a public dashboard within days. Residents and emergency responders can see exactly which neighbourhoods face the greatest risk.
How Wastewater Drug Testing Works
Wastewater drug testing relies on a simple but powerful idea. Whatever a community consumes eventually passes through its sewage system. By analysing that sewage, scientists can detect minute traces of drugs and their metabolites, even in tiny quantities.
Fentanyl, for example, is often used in amounts equivalent to only a few grains of sand. Once metabolised by the body and flushed away, it becomes diluted in millions of gallons of water. That water already carries waste from homes, hospitals and industrial sites. Despite this dilution, modern laboratory techniques can still identify the original drug. They can also distinguish it from fentanyl that has simply been discarded whole.
The approach is not new. Testing wastewater for disease dates back to efforts to contain cholera in nineteenth century London. Monitoring sewage for illicit drugs, however, is a much more recent idea. It gained momentum first in Europe before spreading to American cities.
Sewage Drug Monitoring Offers Communities an Early Warning
Tempe became the first municipality in the United States to adopt sewage drug monitoring in 2018. The city partnered with researchers at Arizona State University. It now samples across its entire forty square miles. Officials deliberately avoid focusing on any single neighbourhood, aiming instead to build public trust.
That broad approach has already changed how officials understand drug use locally. Tempe city manager Rosa Inchausti explained that testing revealed drug traces were not confined to areas associated with homelessness. They also appeared in affluent neighbourhoods. There, residents might otherwise hesitate to call emergency services, fearing a visible police or fire vehicle outside their home.
Wastewater drug testing has already prompted practical responses elsewhere. When xylazine appeared in Tempe sewage, emergency medical staff received additional training to recognise and respond to its effects. In Laredo, Texas, positive results for opioids and stimulants led public health workers to canvass high risk neighbourhoods. They handed out educational leaflets about local support services.
The federal government has also taken note. Last month, the White House Office of National Drug Control Policy highlighted the value of wastewater testing. It can reveal national patterns of illegal drug use and trafficking in close to real time. A year long project is now underway, testing for twenty different substances across roughly one hundred sites nationwide.
Schools Face Difficult Questions Over Sewage Drug Monitoring
The expansion of sewage drug monitoring into schools has raised particular concern, since most students are minors. Missouri pilot testing covered thirty seven high schools this year. Nitazenes, a potent class of synthetic opioids, turned up at twenty six of them.
One of those schools belonged to the small rural Craig R III district. After learning of the result, superintendent Matt Copeland alerted neighbouring school leaders. He arranged for overdose reversal medication to be kept on site. He also invited a state trooper to address the entire town at a public assembly.
Although some in his community questioned his decision to allow testing, Dr Copeland said the resulting education had real value. He believes residents in his town can now discuss nitazenes more knowledgeably than most people elsewhere in the country.
Not every result has proved straightforward to interpret. Occasionally, hydrocodone, a prescription pain medication, appeared in samples from Dr Copeland school. Some teachers later approached him privately. They explained they held a legitimate prescription, apologising for disrupting the results. A separate 2023 pilot tested twenty four New Mexico schools. Ninety two percent of samples came back positive for methamphetamine. Officials later acknowledged that chemists could not yet distinguish the illegal drug from prescription medications used to treat attention disorders.
Privacy Concerns Surround Wastewater Drug Testing
Despite its public health value, wastewater drug testing sits at an uneasy intersection between health policy and law enforcement. Legal scholars generally agree the practice is constitutional. They point to a 1988 Supreme Court ruling that found no expectation of privacy once rubbish is left for collection. By similar reasoning, sewage discarded down a drain carries no such protection either.
Even so, some critics worry that positive results could eventually narrow down individual households, rather than simply track broader trends. Tennessee Bureau of Investigation officials have suggested that focused sampling could help identify a small cluster of properties. Investigators could then examine that cluster further using other intelligence. Unlike Tempe, Tennessee investigators have not made their results public. That choice raises additional questions about transparency.
Natalie Ram, a law professor at the University of Maryland, has written extensively about these tensions. She points out that public health responses aim to identify and help people who are unwell. Law enforcement responses, by contrast, are ultimately designed to identify and punish them. Balancing those two goals responsibly will likely shape how wastewater drug testing develops in the years ahead.
Wastewater Drug Testing Expands Nationwide
Despite the unresolved questions, momentum behind wastewater drug testing continues to build. Sewage samples have already been collected from truck stops in Kentucky and from Mardi Gras celebrations in New Orleans. Numerous school districts nationwide have joined the effort too. Legislators in at least five states have proposed new funding for the practice.
Researchers are also looking ahead. Environmental chemist Bikram Subedi at Louisiana State University has identified cychlorphine, a newly emerged opioid from China. He found it within wastewater collected from a small community in East Tennessee. His team is now exploring whether artificial intelligence could help predict overdose trends. It might even anticipate which substances enter local drug supplies next.
The Phoenix metropolitan area is home to more than five million residents. Tempe currently remains the only city there conducting sewage drug monitoring, despite sharing the same drug supply routes as its neighbours. City manager Rosa Inchausti has begun asking a pointed question of regional partners. If Tempe can identify emerging drug threats early enough to respond, she asks, why should surrounding communities wait to do the same.
For now, wastewater drug testing offers something rare in the fight against dangerous drug trends, a genuine head start. Whether that early information translates into stronger prevention efforts may depend less on the science itself. It will likely depend more on how responsibly officials choose to use it.
