A new prescription opioid safety tool is changing how people manage overdose risk at home. The Opioid Safety Toolkit, an interactive online resource, has shown strong results in a randomised controlled trial published in the journal Addiction. It significantly increased the number of people who requested naloxone, the medication that reverses opioid overdose.
What the Opioid Safety Toolkit Research Found
More than twice as many participants who used the Opioid Safety Toolkit requested naloxone within four weeks. The rate was 21.7% compared to 9.9% for those who used a standard information website. That is an odds ratio of 2.5 (P = 0.005).
The intention figures were even more striking. Shortly after using the toolkit, 41.4% of participants said they planned to obtain naloxone. In the control group, only 15.4% said the same. That is nearly three times the rate, with an odds ratio of 3.9.
Knowledge scores improved substantially too. Participants in the toolkit group scored a mean of 16.6 on a validated overdose knowledge scale immediately after the intervention. The control group scored 13.3. These gains held at the four-week follow-up, with the toolkit group still showing higher scores.
Who Took Part in the Study
The trial enrolled 314 Australian adults prescribed opioids for non-cancer pain. All had taken opioids in the past four weeks. Participants had a mean age of 49.1 years and around 77% identified as female. The median duration of opioid use was 7.4 years. Roughly one in three screened positive for emerging opioid use disorder symptoms.
Both groups spent similar time with their resource, around 25 to 27 minutes on average. That rules out simple engagement time as the reason for the gap in outcomes.
How the Opioid Safety Toolkit Works
Rather than presenting static information, the Opioid Safety Toolkit guides users through building a personalised Opioid Safety Plan. The plan includes interactive risk information, education on naloxone and how to access it, and prompts to share the plan with others in the home.
The design drew on the Theoretical Domains Framework and Behaviour Change Techniques Taxonomy. These are established tools for identifying what drives or blocks safety behaviour. Researchers used them to select techniques that target those drivers directly.
The toolkit was co-designed with consumers, pain specialists, pharmacists, prescribers and researchers. Multiple workshops and extensive user testing shaped the final product.
Lead researcher Professor Suzanne Nielsen noted that people prescribed opioids often have limited understanding of overdose risk. Yet when given clear explanations, they respond positively. Helping people identify their own risks and act on them works better than pointing them to a webpage.
User satisfaction scores backed this up. The toolkit group rated their resource at a mean of 20.0 out of 28. The control group rated theirs at 18.0 (P = 0.035).
Where Prescription Opioid Safety Gaps Remain
More naloxone requests did not fully translate into more naloxone possession. At four weeks, 16.4% of toolkit users had naloxone in hand, compared to 9.7% in the control group. The difference did not reach statistical significance. The most common reason given by those who missed out was that pharmacies did not have naloxone in stock.
This is a system-level problem no consumer tool can fix alone. Even motivated patients hit barriers at the dispensing counter. The research team highlighted improved pharmacy stocking as a separate priority. Efforts to address this are ongoing.
The intervention also did not increase opioid safety conversations with healthcare providers. Around 44% of participants in both groups spoke to a doctor or pharmacist about opioid safety within four weeks. The authors attributed this partly to the active control website, which itself encouraged those conversations.
Why Prescription Opioid Safety Matters
Prescription opioid harm is a serious public health issue across many countries. In the United States, a quarter of the population used opioid analgesics in 2023, according to the Substance Abuse and Mental Health Services Administration. Australia has faced similar trends over the past two decades.
Past approaches leaned heavily on healthcare professionals to start difficult conversations. Many of those conversations never happened. Pharmacist-led naloxone programmes have shown results, but are triggered infrequently. Pharmacists report uncertainty about which patients to target and discomfort raising the topic.
This trial adds to growing evidence that equipping consumers directly with opioid safety knowledge produces real behaviour change. Interactive tools that deliver personalised feedback outperform passive websites. The effect is meaningful, and the cost of digital delivery is low.
Looking Ahead
The four-week follow-up window means long-term sustainability remains an open question. The researchers called for future studies to test whether gains persist and whether ongoing content or reminders could extend them.
The Opioid Safety Toolkit is publicly available at saferopioiduse.com.au. Even modest increases in naloxone uptake at population scale could prevent deaths. That makes widening access to this kind of resource a practical and urgent next step.
