Illicit Drug Use in England Spikes at Heatwaves, Festivals and Football Matches
A major scientific study has painted one of the most detailed pictures yet of illicit drug use in England, and the results are striking. From Eurovision nights to summer heatwaves, and from bank holiday weekends to World Cup matches, the nation’s wastewater has quietly recorded the scale and rhythm of drug consumption in ways that surveys and police reports simply cannot match.
Imperial College London led the research, funded by the Home Office, analysing more than 1,700 water samples from 15 treatment plants across England throughout 2022. Those sites together serve roughly 21% of England’s population. Published in the journal Addiction in 2026, the findings should concern anyone who cares about public health.
Cocaine Dominates, But Ketamine Is the Standout Concern
Of the 20 substances tested, researchers detected 18 in at least one sample. Cocaine registered the highest average daily consumption across all sites at 2,770 mg per 1,000 people per day. Heroin, ketamine, amphetamine, MDMA and methamphetamine followed in descending order.
Cocaine and MDMA showed clear recreational patterns, peaking sharply at weekends. Across all 15 sites combined, the data captured the equivalent of approximately 12 tonnes of cocaine over the year.
Ketamine, however, is what researchers flagged as particularly alarming. Unlike cocaine, ketamine showed almost no variation between weekdays and weekends. “People are using it every single day and to the same degree, no matter whether it’s a weekend or a weekday,” said Professor Leon Barron of Imperial College London’s Environmental Research Group. “It’s becoming very routinely used and it’s really quite a worrying situation.”
Seven English sites recorded higher average ketamine levels than any other monitored location across Europe during the same period. Researchers compared English sites statistically against 109 other wastewater treatment plants across Europe. Cocaine and ketamine levels in England proved significantly higher (cocaine: adjusted P = 0.000115; ketamine: adjusted P = 0.0000389). Amphetamine, MDMA and methamphetamine showed no significant regional differences.
Wastewater Drug Surveillance Captures Events and Celebrations
One of the most compelling findings from this wastewater drug surveillance programme is how closely consumption tracked recognisable national moments.
England played five FIFA World Cup matches in Qatar between November and December 2022. Every single game generated at least one consumption spike across the six sites with available data, with 68 outliers recorded across the entire tournament period.
The Eurovision Song Contest final on 14 May 2022 also left a measurable trace. Five locations provided data on the night. Two showed outliers for cocaine, and one recorded an outlier for methamphetamine. The following day, three of six available sites showed spikes in cocaine and MDMA, with two more flagging ketamine and methamphetamine.
Bank holidays produced statistically significant rises too. Cocaine consumption on bank holiday Fridays and Mondays ran approximately 21% higher than on regular weekends (P = 0.021 and P = 0.020, respectively). MDMA followed a similar pattern, with elevated readings on Fridays, Saturdays and Mondays during bank holidays.
Halloween was particularly notable. The day after the bank holiday Monday produced an extreme amphetamine outlier at one site, recording 7,165 mg per 1,000 people per day. Multiple sites also flagged spikes in MDMA, cocaine, ketamine and methamphetamine around the same date.
How Illicit Drug Use in England Rises During Heatwaves
The summer of 2022 added another dimension to the picture. One southern England site recorded notably elevated cocaine and MDMA consumption on 16 and 17 July 2022. Those dates coincided with one of the strongest UK heatwaves on record at that location.
Hot weather matters here because drug-related health risks grow sharply with temperature. Studies have shown that elevated ambient temperatures increase the risk of fatal cocaine overdose. Wastewater drug surveillance now gives public health officials a precise tool to monitor when and where consumption rises during heatwaves, enabling more targeted emergency planning.
A Drug Seizure Visible Hundreds of Miles Away
Perhaps the most striking finding concerns what happened after a landmark law enforcement operation.
On 17 March 2022, Border Force agents intercepted 3.7 tonnes of cocaine hydrochloride at Southampton Docks, one of the largest seizures in recent UK history. Eight sites held data for both February and March. All eight showed a fall in cocaine consumption, with drops ranging from 14% to 74%.
Three catchments roughly 200 kilometres apart, serving a combined population of around 5.3 million people, all recorded a marked and sustained decline in the weeks that followed. Most sites had returned to average levels by April.
“You can knock out or massively impact cocaine consumption in major urban areas, and these sites were hundreds of miles away from each other,” said Professor Barron.
Alcohol and Cocaine: A Dangerous Combination Tracked by Wastewater Drug Surveillance
The study also tested for cocaethylene, a compound the human body produces only when cocaine and alcohol enter the system together. Levels in wastewater rose significantly at weekends across the majority of sites, pointing to widespread co-consumption of both substances during recreational periods.
The health consequences are serious. Scientists have linked cocaine and alcohol taken together to heightened cardiovascular stress, increased risk of acute coronary syndromes and mental health complications. The wastewater evidence suggests this behaviour is far from rare.
What Illicit Drug Use in England Really Costs
The practical implications of this surveillance are hard to ignore. Wastewater based epidemiology captures the actual biochemical trace of drug use at a population level, in near real time, without relying on self-reporting.
Emergency services can use the data to plan around high-risk events. Public health teams can identify which drugs are most normalised in particular communities. Policymakers can see, in real numbers, how a single large seizure ripples through consumption patterns hundreds of miles away.
The authors caution that the 15 sites studied were predominantly urban and not representative of the whole of England. National level extrapolation is not recommended. North and south England showed no statistically significant differences in consumption across the five drugs tested.
Even so, the backdrop is sobering. In 2022, England and Wales recorded 4,907 drug poisoning deaths, the highest number since 1993. Nearly half involved an opiate. Cocaine-related deaths rose for the 11th consecutive year, reaching 857. The annual cost of tackling drug harm across England and Wales is estimated at £19 billion. Against that scale, knowing exactly when and where illicit drug use in England spikes is not an academic exercise. It is an urgent public health tool.
