GLP-1 Medications Linked to Sharply Lower Rates of Substance Use Disorders

A major new study has found striking results around GLP-1 addiction risk. People taking drugs like Ozempic face substantially lower odds of developing alcohol, opioid, nicotine and cocaine dependence. These findings raise serious questions about what weight loss drugs are actually doing inside the brain.

Pharmacologists Tadesse Abegaz and Gabriel Frietze at the University of Texas at El Paso led the study. Their paper appeared in Frontiers in Psychiatry. The dataset covered more than 142,000 patients with type 2 diabetes or obesity. Around 20,000 of those patients took GLP-1 receptor agonists. Substance use outcomes were then tracked and compared across the full cohort.

GLP-1 Addiction Risk: What the Numbers Show

The findings were notable across every substance category. People on GLP-1 medications had 74 per cent lower odds of alcohol use disorder. They had 75 per cent lower odds of cocaine use disorder. Opioid use disorder odds were 69 per cent lower. Nicotine use disorder odds were 68 per cent lower. All figures held against matched patients not taking the drugs.

Earlier research into GLP-1 receptor agonists and substance use drew on smaller studies and animal models. The All of Us Research programme supplied patient data for this analysis. It is one of the most diverse health databases in the United States, covering more than one million participants. This study goes considerably further than what earlier observations could offer.

How Brain Chemistry May Explain the Effect

Scientists believe the link between GLP-1 addiction risk and neurological function lies in dopamine signalling. These drugs originally targeted blood sugar regulation and appetite control. However, they also appear to affect the neural circuits involved in reward and craving. The same mechanism that reduces food-seeking behaviour may similarly dampen responses to alcohol and drugs.

Lead researcher Tadesse Abegaz pointed to this directly. He said the findings build on growing evidence that these drugs influence far more than appetite and blood sugar. The brain pathways they affect appear to overlap with those driving addiction. That overlap could account for the lower GLP-1 addiction risk observed across the study population.

Researchers Urge Caution on Clinical Use

Despite these results, the research team drew firm boundaries around what their data actually proves. This was an observational study in a specific clinical population. It shows an association, not cause and effect. Furthermore, the researchers do not support prescribing these drugs for addiction treatment at this point.

Co-author Gabriel Frietze was clear on this. Randomised clinical trials must come first. He said GLP-1 receptor agonists and substance use outcomes cannot shape formal treatment protocols until that evidence exists. Observational findings, however large, carry confounding variables that only trials can rule out.

Patients taking GLP-1 medications may also differ from the comparison group in lifestyle or socioeconomic background. Those factors could independently reduce substance use disorder risk, separate from any drug effect.

What Comes Next for This Research

The UTEP team plans prospective studies that follow patients from the point they begin GLP-1 therapy. Abegaz said the goal is to track whether substance use behaviours shift after treatment starts. The team also wants to assess whether any such changes connect to improvements in mental health and quality of life.

The core question is whether lower GLP-1 addiction risk holds up under controlled trial conditions. If it does, these drugs could eventually shape new approaches to supporting people with addiction. That remains, for now, a question rather than a conclusion.
