Codeine sits in millions of bathroom cabinets across the UK. Most people treat it as a routine painkiller. Yet the codeine risks are real, poorly understood, and shaped by factors most users never consider, including their own genetics. Behind the familiar packaging lies a far more complicated story.
Understanding those risks is not just a concern for doctors and pharmacists. It matters to anyone who reaches for a packet of co-codamol and assumes it is straightforward.
What Codeine Actually Does in the Body
Codeine is an opioid. Doctors prescribe it for mild to moderate pain, and some formulations use it to suppress coughing. Over-the-counter versions combine it with paracetamol or ibuprofen. Stronger doses require a prescription.
By itself, codeine is a relatively weak opioid. Its pain-relieving effect is roughly one tenth that of morphine. The liver converts part of it into morphine, which then acts on opioid receptors in the brain. For most people, this works as expected. For a significant number, it does not.
Codeine Risks Start With Your Genes
Genetics plays a much bigger role in codeine risks than most people realise. The enzyme CYP2D6 controls how the body converts codeine into morphine. It does not work the same way in everyone, and the differences matter enormously.
Around 1% to 2% of people are ultra-rapid metabolisers. Their bodies convert codeine into morphine far faster than average. Even a standard dose can push morphine levels dangerously high. This genetic trait is more common among people of North African and Middle Eastern descent. The result can be severe drowsiness, breathing difficulties, and in serious cases, life-threatening toxicity.
Between 5% and 10% of people sit at the opposite end. These poor metabolisers convert very little codeine into morphine. The drug gives them almost no pain relief. Poor metabolism appears more often in people of white European descent. When relief never fully arrives, some people take more. That pattern feeds directly into codeine dependence.
A further 2% to 11% of people fall somewhere in between as intermediate metabolisers. They get limited benefit from the drug. Most people have no idea which group they belong to.
When the Numbers Do Not Stack Up
The genetics problem becomes worse when you look at the evidence on effectiveness. Products containing less than 10mg of codeine deliver only modest pain relief. They still carry opioid-related risks. That is not a good trade-off.
A review of clinical trials found that low-dose codeine combinations gave limited benefit for short-term pain, such as dental pain or post-operative discomfort. Many of those underlying studies were also too small to be conclusive. Notably, research shows that paracetamol combined with ibuprofen can match or outperform low-dose codeine, with none of the opioid-associated codeine risks.
Higher-dose formulations combining ibuprofen with 25mg to 60mg of codeine do appear more effective. Doctors prescribe these only for a reason.
Understanding Codeine Dependence
Codeine dependence can develop even when someone takes the drug exactly as directed. The brain’s opioid receptors adapt to the drug over time. Researchers call this process tolerance. As tolerance grows, the same dose produces less effect. The temptation to take more follows naturally. Tolerance can begin developing within just a few days.
Once the body becomes physically dependent, stopping abruptly is not easy. Withdrawal brings restlessness, sweating, anxiety, and poor sleep. For some people, these symptoms become the main reason they keep taking codeine, long after the original pain has gone.
Even at normal doses, side effects are common. Constipation, nausea, dizziness, and drowsiness affect many users. Higher doses slow breathing and impair coordination. Codeine also interacts with sedative medicines. Some antidepressants block the CYP2D6 enzyme entirely, making codeine less effective and pushing users toward higher doses, which increases codeine dependence risk further.
Codeine Risks That Led Regulators to Act
UK regulators did not tighten the rules around codeine without reason. The Medicines and Healthcare products Regulatory Agency now requires clearer addiction warnings on all packaging. Over-the-counter packs carry a 32-tablet maximum. Non-prescription codeine products carry a recommended limit of three days of use.
Codeine linctus tells its own story. In 2023, regulators moved it to prescription-only status after growing evidence of misuse. People had been mixing the cough syrup with soft drinks and sometimes alcohol in a preparation known as “purple drank.” Its opioid content caused dependence, breathing problems, and overdose. That reclassification was not a surprise to those who had been watching the numbers.
These are not the characteristics of a harmless cough remedy. They are the hallmarks of a drug that demands more careful handling.
Weighing Up the Codeine Risks Before You Reach for the Packet
Codeine still has a place in pain management. For short-term use, under proper guidance, it can help when other options fall short. That is not in question.
What is worth questioning is the casual familiarity most people bring to it. The codeine risks vary depending on the dose, the individual’s genetic profile, and how the drug fits with other medicines they take. Two people can take the same tablet and have completely different outcomes, one finding modest relief, the other facing a difficult dependence to untangle.
Anyone who finds stopping harder than expected should treat that difficulty as a signal, not a coincidence.
