A new study may have found the primary route through which alcohol use disorder in families passes from one generation to the next. Researchers point to a specific brain region involved in how people process reward. The findings could change how we approach prevention for young adults at risk.
The research was published in Alcohol, Clinical and Experimental Research in 2026. It found that young adults with a family history of the condition showed heightened activity in the nucleus accumbens. This is a brain region linked to reward motivation. That heightened activity predicted steeper increases in harmful drinking over the following three years.
What the Research Involved
Researchers at the University of Florida, led by Amanda Elton, recruited 163 first-year college students aged 18 to 19. All had varying levels of familial alcohol use disorder risk. Each participant underwent functional MRI scanning. During the scan, they completed a decision-making task. They chose between smaller amounts of money available immediately or larger sums available later. This exercise is known as delay discounting. It measures how strongly a person is drawn to immediate rewards over delayed ones.
Participants also completed drinking surveys at the start of the study. They then repeated these annually for three years, using the Alcohol Use Disorders Identification Test (AUDIT). Scores of 8 or above are associated with harmful drinking.
Having a close biological relative with alcohol use disorder raises personal risk by three to four times. Around 22% of adults in the United States report having a biological parent with the condition. Understanding how that risk transmits across generations has become a research priority.
Alcohol Use Disorder in Families and the Nucleus Accumbens
The results were striking in their specificity. Three brain regions were examined. These covered areas linked to reward, future-oriented thinking, and cognitive control. Only the nucleus accumbens showed a significant relationship with both family history and later drinking patterns.
Participants with a family history of alcohol use disorder showed greater nucleus accumbens activation during decision-making. That elevated activation predicted a steeper rise in alcohol misuse scores over the study period. This held true even after adjusting for childhood trauma, prior binge drinking, and depression.
The indirect pathway from family history through the nucleus accumbens to worsening alcohol misuse was statistically significant (β = 0.147, p = 0.044). This suggests the reward system may act as a conduit. Through it, inherited or environmental vulnerability may translate into riskier drinking behaviour.
Behavioural measures of delay discounting alone did not significantly mediate the link. Brain imaging offered explanatory power that observed behaviour could not. What happens beneath the surface in terms of neural activity may be a more sensitive early marker of risk.
Why Reward Sensitivity Matters
The nucleus accumbens sits at the heart of the brain’s central reward circuit. When overactive during decision-making, it may lead individuals to assign greater weight to immediate rewards. In practical terms, alcohol can feel more compelling in the moment. Short-term relief outweighs perceived longer-term consequences.
Previous research had already found structural differences in the nucleus accumbens among adolescents with familial alcohol use disorder risk. Altered resting connectivity patterns were also noted. This study adds a functional dimension. It links those neural differences to real-world drinking outcomes tracked over four years.
The hippocampus and the middle frontal gyrus did not emerge as significant mediators in this sample. These regions may play a larger role at different life stages or in clinical settings.
Implications for Preventing Alcohol Use Disorder in Families
For those working in prevention, the findings open a meaningful new avenue. Rather than relying on self-reported family history alone, brain imaging could one day identify at-risk young people before harmful patterns take hold.
The researchers suggest individuals with elevated reward-related reactivity could benefit from tailored approaches. These might target reward sensitivity directly or increase the appeal of healthier alternatives. This kind of precision-based strategy accounts for individual neurobiology. It moves beyond blanket group-level programmes.
Current guidance from the US National Institute on Alcohol Abuse and Alcoholism focuses on group-level approaches. These include targeting students in Greek organisations. None currently use individual neurocognitive profiles to guide intervention. This study suggests such an approach may be worth pursuing.
Interestingly, the mesolimbic dopamine system, of which the nucleus accumbens is a core part, has also been linked to how natural rewards can protect against developing alcohol use disorder. Strengthening the appeal of non-alcohol rewards may be a viable prevention strategy for young adults with familial risk.
Limitations and Next Steps
The sample was drawn from college-educated young adults in one region of the United States. Alcohol misuse may be driven by different neural mechanisms in older populations or clinical groups. There was also some dropout at later follow-up points. However, baseline drinking levels did not predict who dropped out.
The study was not powered to examine sex differences in the role of family history and brain function. The researchers flag this as a priority for future work.
That said, the nucleus accumbens finding held up across multiple sensitivity analyses. Models adjusted for anxiety, depression, and childhood adversity all returned the same result. The reward system looks like a robust marker of familial alcohol use disorder risk in young adulthood.
Getting to grips with how alcohol use disorder in families transmits between generations is not simply a scientific exercise. It brings us closer to identifying who is most vulnerable, early enough to act.
